From referral to Form 3

A five-step process with a named contact throughout and progress updates you don’t have to chase. Standard, priority and urgent turnaround tiers are available on every referral.

  1. Referral received

    Send Form 1/Form 4 documentation by secure email. We acknowledge the same working day.

  2. Assessor allocated

    A qualified BIA local to the placement is allocated, normally within one working day.

  3. Assessment arranged

    The assessor contacts the managing authority, the person and their representatives to arrange visits.

  4. Assessment completed

    Face-to-face assessment, consultation with family and professionals, and full record review.

  5. Report delivered

    A quality-assured Form 3 delivered within your commissioned timescale — standard or urgent.
